The Senegalese international came back to haunt former boss Ronald Koeman as he found the back of the net after Daniel Sturridge's shot had come back off the post.
The two sides played out a fiery encounter before the Senegalese star tucked home four minutes into added time.
Roberto Firmino had gone the closest of the two sides before substitute Daniel Sturridge hit the post and Mane tucked in the rebound.
Everton , who lost James McCarthy and Martin Stekelenburg to injury, were fortunate to end with 11 men after a horror tackle from Ross Barkley on Jordan Henderson.
A lot was made of Everton's 'Dogs of War' approach, similar in the win over Arsenal, in the build-up to this one.
They made life very difficult for Liverpool again for the large part before tiring after the break.
Ronald Koeman has got his side playing again, simply by running harder and faster, while Liverpool's effort and work-rate can never be underestimated.
This was a throwback to the old days in the derby with tackles flying in across the pitch but there was a real lack of quality.
It appeared to be enough for a point for the Toffees until Mane finally broke their resistance.
